The PACT (Patient Aligned Care Team) Licensed Practical Nurse provides patient care through an integrated primary care concept, and each veteran is assigned to a team of health care providers who follow the patient's care both as an inpatient and an outpatient. He/she models the interdisciplinary team approach to patient care and involves others in this process. He/she provides education to patients, families and significant others; administers medications and treatments; participates in the quality improvement process; and is attuned to the needs of the patients. Reports to the ambulatory care Nurse Manager.

Work Schedule: Primarily Monday-Friday, 8:00 am - 4:30 pm, but may be required to work other shifts and weekends if needed for patient care.

Basic Requirements: United States Citizenship.

Graduation from a school of practical nursing approved by the appropriate State accrediting agency or the Accreditation Commission for Education in Nursing (ACEN) at the time the program was completed.

Current, active and unrestricted LPN license from any U.S. state or territory.

Current Basic Life Support (BLS) certification.

Preferred Experience: Minimum of 3 years' experience in Medical/Surgical nursing. The Licensed Practical Nurse should be computer literate, flexible, customer friendly, and have the ability to multi task. Incumbent must be a self starter that can work independently and perform complex assignments. He/she must be motivated and possess excellent organizational skills. Dependability is a must. He/she must adapt quickly to change beyond the traditional role assignment. He/she must be able to work with administrative and interdisciplinary team members.

Grade Determinations:

GS-3 - None beyond the basic requirements. LPN's at this grade level are expected to perform all duties within the scope of their license; however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved.

GS-4 - Six months of qualifying experience as an LPN or LVN; or graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient care clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or Graduation from an approved school of at least 24 month duration.

GS-5- Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent and demonstrate knowledge and ability to perform a full range of practical nursing care, ability to serve as a responsible member of the nursing team, sufficient to prepare, administer, and appropriately document actions.

GS-6-Completion of at least 1 year of qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements. Demonstrate technically proficient in initiating, performing and competing assigned duties in providing care to variable patient populations.

The full performance level of this vacancy is GS-6. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-3 to GS-6.
