The VA Palo Alto Health Care System has an immediate opening for a full-time Ambulatory Care Pharmacy Manager. The primary function of this position is to provide oversight of the pharmacy services for the ambulatory care section across the health care system. This includes personnel management of 17.0 FTEE Ambulatory Care Clinical Pharmacy Specialists (CPS) and 1.0 Clinical Pharmacy Technician staff located throughout six (6) clinic geographic locations, as well as ambulatory care pharmacy program management. The incumbent will report directly to the Associate Chief, Pharmacy Service.

The incumbent is responsible for planning, directing and coordinating the ambulatory care clinical program associated with providing independent practitioner services in Patient Aligned Care Teams (PACT), Anticoagulation, and Home Based Primary Care (HBPC). This includes a broad spectrum of disease state management under individual pharmacist scopes of practice including, but not limited to: diabetes, hypertension, hyperlipidemia, anticoagulation, heart failure, COPD, pain and geriatric deprescribing.

This program manager serves as a subject matter expert within Pharmacy Service and with other facility health care staff and all levels of hospital management regarding ambulatory care clinical services. This includes facilitating collaboration and cooperation with other health care professionals and managers in promoting an interdisciplinary approach to patient care.

The major duties and responsibilities include, but are not limited to: Direct supervision of employees; including performance and personnel management.

Conducting process improvement and/or quality assurance projects to assess program performance based on national, VISN and local performance measures.

Developing long range program planning and development for ongoing program relevance and growth, assuring implementation, maintenance and compliance with VHA guidelines and standards of professional practice.

Basic Requirements:

U.S. citizenship.

Education

(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy). [ TRANSCRIPTS MUST BE SUBMITTED WITH APPLICATION
