Responsibilities

The Senior Social Worker serves as the program coordinator at the and is administratively responsible for the Homeless Patient Aligned Care Team (HPACT). The Senior Social Worker also serves as the social worker for HPACT, and as a Grant and Per Diem (GPO) Liaison for community based transitional housing programs for homeless Veterans.

The Senior Social Worker:

> Practices in the Health Care for Homeless Veterans (HCHV) Program and serves a patient population with highly complex health and mental health problems.

> Provides direct care services and manages the daily operation of the HPACT.

> Develops policies and procedures for program operation and prepares reports and statistics for the facility, Veterans Affairs Integrated Service Network (VISN), and national use.

> Is responsible for oversight of the program's budget arid various control points assigned to the program.

> Manages and controls the use of data related to the day to day functioning and program evaluation.

Plans, organizes and directs administrative, operational, and patient and personnel activities for the HPACT

In conjunction with the Social Work Supervisor

Additional responsibilities of this position are to:

> Provide oversight of the clinical and administrative services provided by the G&PD provider;

> Provide case management services to the clients residing in the G&PD program and for Veterans who are enrolled in the HPACT;

> Utilize various treatment modalities of individual and group work, casework, community organization, and community outreach;

> Coordinate and participate in G&PD site inspections,

> Participate in program planning and development, and process improvement activities;

> Participate in annual community based events.

> Conduct outreach to homeless Veterans *Candidate must be able to work with city/county agencies to increase resources for on-the-street or marginally housed Veterans.

The Senior Social Worker is responsible for:

> Key project activities related to the planning, development, and implementation of the HPACT

> managing fleet use regarding government vehicles;

> coordinating the team staff meetings and the care huddles;

> managing equipment requests;

> overseeing the HPACT consult process and clinic schedule.

> providing consultation and guidance to colleagues *must exhibit role model effective social work practice skills,

> teaching or providing orientation to less experienced social workers,

> developing innovations in practice interventions

> providing clinical supervision for social work licensure or certification.

Work Schedule: Full-Time/40 Hours per Week. Work Schedule will be determined based on clinic need, the employee preference.

Qualifications

BASIC REQUIREMENTS:

· U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

· Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.

· Licensure. You must be licensed or certified by a state at the Advanced Practice Level which included ASWB advanced generalist or clinical examination, unless you are grandfathered by the state in which you are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice. (Must provide a copy of your licensure or certification)

·Loss of Licensure or Certification.

Once licensed or certified, social workers must maintain a full, valid and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-185 social work series and may result in termination of employment.

Preferred Experience

Highly qualified candidates will have the following:

> Experience with a population that is Homeless or at risk for being homeless

> Experience working with community agencies, community partners and stakeholders.

> Experience working with clients in the community and in their home.

> Experience providing Mental Health Services and or addiction services with high acuity clients.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

GRADE DETERMINATIONS:

GS-12 Experience, Education and Licensure. Senior Social Workers have experience that demonstrates possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice ost Masters of Social Work degree. Senior social workers must be licensed or certified by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). Experience that demonstrates possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. have experience that demonstrates possession of advanced practice skills and judgment. At least two years are in an area of specialized social work practice, and demonstrate the following.

KSA's:

1. Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex physical or mental illness. Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Ability to determine priority for services and provide specialized treatment services.

2. Advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of veterans with polytraumatic injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, etc.

3. Advanced knowledge and expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services. Ability to coordinate the delivery of specialized psychosocial services and programs. Ability to design system changes based on empirical findings.

4. Ability to provide subject matter consultation to colleagues and students on the psychosocial treatment of patients treated in the specialty area, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. Ability to teach and mentor staff and students in the specialty area of practice and to provide supervision for licensure or for specialty certifications.

5. Ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program.
