The Clinical Pharmacy Specialist (CPS)plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy. As a consultant,for intensive medication therapy management services, the CPS promotes patient-centered, safe, effective, evidence-based,responsible medication use; provision of drug information services and patient/family education; serve as preceptortopharmacy students; involvement in medical center committees; and the development and management of pharmacy programs related to the practice area. The CPS will participate in pharmacological management of patient medication needs, provision of educational services to staff and patients, the development and management of clinical programs and adverse drug reactions, involvement in medical center committees, and ensure effective clinical pharmacy service are provided.The specialist reports to the Associate Chief, Clinical Pharmacy Services and Chief of Pharmacy Services or designee. H/she coordinates, develops, manages and controls the chronic disease state manage clinical pharmacy services, specifically within the Farmington Community Based Outpatient Clinic (CBOC) and other PACT services as needed, caring for rural Veterans in order to increase patient access to clinical services and improve overall patient care. The CPS provides chronic disease state management to PACT patients. Conduct education sessions for patients as appropriate to improve patient adherence and understanding of their medication regimen. Serve as a consultant to other providers for intensive medication therapy management services.

Monday - Friday, 8:00 a.m. to 4:30 p.m.

Basic Requirements:

U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Graduate of an American Council on Pharmaceutical Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.

Preferred Experience:

Completion of an American Society of Health-System Pharmacists accredited PGY-1 Residency program, 1 year of clinical experience beyond PGY-1 training within the last 3 years and Board of Pharmaceutical Specialties certification, CDE, or BC-ADM certification.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience

Grade Determinations:

In addition to the experience requirements and KSAs at the GS-12 you must have one year of experience equivalent to the next lower grade level. Pharmacists assigned to this position must demonstrate thefollowing KSAs:

1.Ability to communicate orally and in writing topersuade and influence clinical and management decisions.

2.Expert understandingof regulatory and quality standards for their program area.

3.Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.

4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.

5. Advanced skill in monitoring and assessingthe outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements:

Work is performed in a locked pharmacy with possible exposure to chemicals; and in patient care areas where there is potential for exposure to communicable diseases and other hazards.Work consists involves direct and indirect interaction with patients, providers and interdisciplinary teams.
