About the Position: This position is located at Fort Belvoir Community Hospital, Fort Belvoir, VA, Department of Administration, Discharge Planning. Discharge Planning Nursing Service is located in the Directorate for Nursing Services, assesses medical and surgical patients' needs to arrange services and resources after hospitalization for a seamless, timely, and safe discharge. The Nurse Consultants (Discharge Planners) arrange skilled home health and custodial service, order Durable Medical Equipment (DME), and coordinate Primary Care Manager (PCM)/Specialty Services appointments. The Nurse Consultants (Discharge Planners) also determine patient needs on the basis of diagnosis, prognosis, and social support system/person information. Other responsibilities specific to Discharge Planning includes contacting the appropriate agencies to meet patients' social, emotional and spiritual needs. Discharge Planning Nursing Service is committed to providing comprehensive family-centered care to all beneficiaries and their family members. The selected applicant for this position can expect to perform a variety of RN duties to include, but not limited to, working directly with a multidisciplinary team of physicians, nurses, social workers, and possibly other health professionals where the goal is to enhance continuity of care. The incumbent must maintain confidentiality when interacting with patients, families, personnel, and the public. This position requires prolonged periods of walking, standing, bending, or stooping, as well as the ability to lift, push, or pull objects, greater than 50 pounds. The position requires that the selected candidate be available to perform rotating shift work. This individual will be designated as Mission Essential and assume Essential Personnel status during Inclement weather and/or emergencies.

This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Tier 3 security investigation or access for the duration of employment. A background investigation and credit check are required.
This position requires the incumbent to have tuberculosis testing.

This position requires the incumbent to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
This position requires that you obtain and maintain a Basic Life Support (BLS) certification.
This position may require the incumbent to work on any day or shift, including PMs, nights, weekends, holidays, or on an on call basis, necessary to meet mission requirements.
This position has been designated Mission Essential. In the event of severe weather conditions or other such emergency type situations, the incumbent is required to report to work or remain at work as scheduled to support mission operations.

Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must submit a copy of your transcripts at the time of application) Or official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. (Note: You must submit a copy of your certification at the time of application)
Licensure: Applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Experience: One year of experience which includes Discharge Planning, Case Management or Utilization Management, providing discharge-planning services for patients with complicated medical-surgical problems, requiring skilled home care services, and/or in-home assistance following discharge. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).
