This RN Home Telehealth Care Coordinator/ Call Center Float RN position is in the Nursing Service, Community Care Section, VA St. Louis Healthcare System (VASTLHCS), St. Louis, Missouri. This position supports both Home Telehealth and the VASTLHCS Call Center. This is a RN who coordinates care for a panel of patients through the continuum of care utilizing Telehealth technologies to assure that care is timely, appropriate, of high quality, and cost effective. The nurse utilizes sound clinical judgment, functions independently with decision making and demonstrates the ability to assume responsibility for the management of complex problems. Within the Call Center, the RN works with a team to provide acute symptom-based management via telephonic triage to Veterans. The nurse works closely with the inpatient and outpatient interdisciplinary clinical teams, other clinics, internal or external services and community agencies. This position delivers efficient, comprehensive, and continuous patient-centric holistic care through active collaboration, communication and coordination of resources. Our facility is seeking self-directed individuals who possess expert organizational skills, critical thinking and decision-making ability to enable the completion of work and the ability to establish effective communication with patient, families, visitors, and members of the interdisciplinary team to facilitate appropriate care.

1. Provides initial and ongoing assessment of patients to identify needs, issues, care goals and appropriate resources necessary for care management. Utilizes approved evidenced-based protocols and algorithms via computerized software to advise and educate the patient, family, or caregiver.
2. Utilizes sound clinical judgment in assessing, planning, implementing, documenting and evaluating patient care.
3. Appropriately documents interventions, individualized care plans, motivational interviewing, coaching to SMART goals and reviews appropriate documentation of patients for treatment and interdisciplinary goal planning.
4. Recognizes emergency situations and demonstrates effective action in emergencies. To include clinical decision support tools, protocols for mental health problems and other emergency resources.
5. Duties include collaboration with the healthcare team to include both internal/external customers to include the Patient Aligned Care Teams and expanded team of family/caregiver, internal and community-based services to discuss, address and advise patients, families, and clinical staff regarding health care issues or decisions.
6. Assumes accountability of performance outcomes which result in timely and appropriate interventions, improve customer satisfaction, alleviate health outcomes of a serious or life threatening nature and reduce unnecessary clinic visits.
7. Utilization of evidenced-based health information and telecommunication to promote therapeutic, collegial and shared decision-making between the customer and supporting interdisciplinary team members; provides health education that is focused on Veteran goals, health prevention and wellness principles and promoting Veteran health and well-being.
8. Demonstrate patient advocacy by delivery of exceptional customer service in support of the individual acute clinical needs and concerns of internal/external customers. Demonstrates the ability to cope with and manage competing priorities to problem solve.
10. Assess patients for use of appropriate Telehealth devices. Follow procedures for ordering through Prosthetics Services, documents installation and education in electronic medical record.
11. Tracks and trends data by interview and review of patient's records.
12. Communicates with key support staff regarding installation, maintenance, troubleshooting of technological equipment used in care coordination and documentation/completion of encounters.

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN
) or The Commission on Collegiate Nursing Education ( CCNE
). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

3. Two years of Medical/Surgical nursing clinical experience
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.

The Physical requirements of this position include the following tasks which may be performed intermittently: lifting of up to 50 lbs. (3 hours), sitting (2 hours), standing (4 hours), walking (8 hours), climbing (1 hour), kneeling (3 hours), bending/stooping (3 hours), twisting (2 hours), pushing/pulling (6 hours), simple grasping (8 hours), fine manipulation (8 hours), reaching above the shoulder (4 hours), and operating machinery (8 hours).
