Duties This Licensed Vocational Nurse (LVN) position is located in our Tulare Community Based Outpatient Clinic under the Primary Care Service at the VA Central California Healthcare System (VACCHCS), Tulare, CA. The LVN is a member of the Patient Aligned Care Team (PACT) and Expanded Team involved in providing care to the patient and in developing the PACT plan. The LVN\/LPN is expected to demonstrate knowledge and ability to vary the order and sequence of assigned procedures based on patient's condition; to use judgement in selecting the most appropriate application of procedures and guidelines; and to accurately report and record significant patient information. Other duties include, but are not limited to: - Observe, identify, and respond to the patient's needs for nursing, medication, and equipment-assisted care and to recognize deviations from normal. In modifying patient care plans, the employee considers the physical, emotional and social factors. - Prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, intradermal, subcutaneous, and\/or intramuscular mediations, immunizations or skin tests as permitted by approved local facility policies and procedures. - Observe and document a patient's response to medications administered and report any change in patient's condition to RN or MD\/DO, as appropriate. Recognize the need for and institute emergency measures when indicated, promptly seek the assistance of the RN or MD\/DO and assist in resuscitation procedures in cardiac and\/or pulmonary arrest. - Adhere to standards of practice and safe technique in preparation and administration of medications. - Complete health nurse procedures, including EKGs, wound care, taking vital signs, ear lavage, etc. - Based on the patient's goals, provide the patient, family and caregiver health education to include self-management, prevention and wellness. - Participate within the PACT team secure messaging framework, typically as triage team member. - Maintain a clean clinic environment and stocking assigned exam rooms daily.
