Licensed Practical Nurse (HBPC)
The VA Northeast Ohio Healthcare System is recruiting for two Licensed Practical Nurses (Home Based Primary Care). One Licensed Practical Nurse (Home Based Primary Care) will function in Home Based Primary Care (HBPC), Community Outpatient Service (COPS) based out of the Painesville Community Based Outpatient Clinic (CBOC) and one will function out of the Wade Park campus.

Making independent home visits to veterans with complex medical and psychosocial conditions.
Making astute observations, thoroughly documenting care, patient education, and the patient's response(s).
Seeking assistance and guidance when needed to inform the Registered Nurse or Medical Provider of changes in the patient's condition that potentially could require a higher level of intervention.
Making at least 4 home visits per day with frequently driving up to distances of over 100 miles per day.
Actively seeking out educational opportunities to enhance nursing knowledge and skills, and sharing new knowledge gained with other staff to improve and advance nursing practice.
Effectively incorporating knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers.
Preparing and administering prescribed medications (oral, topical, subcutaneous, and/or intramuscular) and performing treatments according to established policies/procedures.
Observing, identifying and responding to the patient's needs for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors.
Fostering an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery.
Establishing constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills.
Appropriately carrying out assigned patient care based on the patients' conditions; using judgment in selecting the appropriate order and sequence of procedures and treatments; and accurately recognizing, reporting and recording relevant patient information.
Recognizing urgent or emergent patient care situations, seeking assistance of the RN, MD/DO, and/or EMS, and initiating appropriate emergency interventions as directed.
Appropriately utilizing knowledge and understanding of human behavior, patient motivations, and reactions to situations in working effectively with patients, family members, and other staff.
Working with an Interdisciplinary Treatment Team.

* Must possess a full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
* Clinical experience in an adult medical nursing setting, skilled nursing facility, skilled home care or hospice setting
* Sound history/physical screening experience
* Leadership and critical thinking skills.
* Flexibility as a team member and positive relationship skills on an interdisciplinary team.
* Able to prioritize responsibilities
* Able to work in wide variety of home environments.
GS-07: Must possess at least one year of qualifying experience at the GS-6 level or equivalent. You must fully meet all performance requirements for the GS-6 LPN/LVN or equivalent and your resume must fully support this. Examples of experience include: proficiency in initiating, performing and completing assigned duties in providing care to variable patient populations, knowledge and ability to appropriately carry out assigned patient care based on patients' conditions; selecting the appropriate order and sequence of procedures and treatments; completed work only needs a general review by a registered nurse or physician; preparing and administering prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous); and performing duties for complex diagnostic tests and/or specialized practices.

The full performance level of this vacancy is GS-07.
