The physician serves as the Physician Director for the Community Based Outpatient Clinic. The physician will have both administrative duties and direct patient care duties with 30% of his or her time devoted to administrative duties as CBOC Physician Director and 70% devoted to direct patient care.

Work Schedule: Typically Monday - Friday, 8:00am - 4:30pm, alternative work schedules may be negotiated.

The Physician Director leads provider teams at the CBOC (Community Based Outpatient Clinic). The Physician spends 30% of his or her time performing supervisory functions. This includes (but not limited to) supervising providers ensuring adequate staffing, approving leave, monitoring and giving feedback on productivity data, and ensuring and guiding the clinic staff/providers to meet various clinical performance measures and access requirements. Other duties include: assisting the Chief of Primary Care and facility leadership in meeting various VHA requirements, assisting in meeting various regulatory agencies' (like OIG, JC and others) standards, and addressing customer care related issues at the CBOCs. Finally, the incumbent will be involved in at least one local Quality Improvement project per year.

The physician spends 70% of his or her time as a PCP (Primary Care Physician). The CBOC operates under a PACT Model (Patient Aligned Care Team). Together with his or her team, comprised of a PCP, RN, LVN, and a Clerk, the physician cares for a panel of approximately 700 patients. The PACT provides comprehensive care for our Veteran population, including the diagnoses, care, and treatment of both acute and chronic conditions. Common diagnoses encountered include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain. The incumbent will also lead the PACT in coordinating care of the veterans in non-face to face or group settings with team members.

Core duties to include: obtaining patient histories, performing physical exams, formulating a working diagnosis and developing/implementing a treatment plan according to clinical guidelines; determining appropriate hospitalization, and/or disposition to Nursing Home Care, Assisted Living, Community Nursing Home, and/or Home Care. Ordering lab tests, x-rays, EKG's and other diagnostic procedures. Initiating or expediting requests for consults, special tests, or studies. Providing education and counseling to patient and family in preventive care, medical conditions and use of prescribed treatment and drugs. The incumbent will assess, stabilize, and determine disposition of patients with emergent conditions consistent with medical staff policy regarding emergency and consultative call services. The physician may collaborate with/oversee mid-level providers such as nurse practitioners, and works collaboratively with other specialties in and outside of the VA system to manage or co-manage patients' health. Select mental health and subspecialty services are available on-site. Other mental health, medical and surgical subspecialties services are available through consult to the San Francisco VA Medical Center or to the community.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA policy.

Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.

Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.

Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.

Proficiency in spoken and written English.

2. Experience as a primary care physician in the VA System

3. Experience with an electronic medical or health record

4. Experience managing and supervising

Physical Requirements: Applicants must meet physical standards for the position. A physical examination prior to placement is required. This is a designated drug testing position. After appointment, you will be subject to random testing for illegal drug use. Requires lifting 15-44 pounds; pushing (approx. 2 hours); reaching above shoulder; use of fingers and both hands; walking and standing from 3-5 hours and kneeling. Ability for rapid mental and muscular coordination simultaneously. Must have depth perception and ability to distinguish basic colors and shades of colors. Hearing aid is permitted.
