Responsibilities
The Homeless and Justice Clinical Recovery (HJCR) Program includes the following sections: Community Employment, Health Care for Homeless Veterans (HCHV) Outreach, Grant and Per Diem (GPD), Homeless Patient-Aligned Care Team (H-PACT), Housing and Urban Development - Veterans Affairs Supportive Housing (HUD-VASH), and Veterans Justice Outreach (VJO). The program provides services throughout the State of Utah, as well as in Southeastern Idaho and East Central Nevada. The incumbent's duties, include but are not limited to:
Assists the HJCR Program Director to oversee the day-to-day operations and management of the program.
In the absence of the Program Director, the Assistant Manager makes decisions on work problems presented by supervisors and/or staff throughout the medical center.
Participates in CoC meetings and planning efforts which may include leadership roles within the CoC group.
Provides direct linkage from community services to VHA homeless program services through regular and consistent outreach and communication with community providers. This includes, but is not limited to, direct linkage to VA mental health services for those Veterans who are at high risk for suicide.
Participates in case conferencing process within the CoC's Coordinated Entry System to help inform and facilitate referrals to VA programs such as HUD-VASH, SSVF, HCHV and GPD.
Ensures efficient sharing of Veteran data and program information, as allowable under VA Privacy and Information Security policies and Directives.
Participates and contributes to a CoC level resource-and-demand analysis, including periodic review of the gaps to determine inflows/outflows, and make recommendations to VHA homeless program leadership on adjustments to resource allocations within coordinated entry based on this analysis.
Provides support to the VA homeless program teams as a functional member of the team, to include participation in outreach activities and subject matter consultation on community involved interventions for homeless Veterans.
Serves as a member of an interdisciplinary homeless program treatment team to link treatment team discussions to the community's case conferencing discussions, ensuring continuity of care for Veterans experiencing homelessness in addition to complex physical and mental illnesses, including those at risk for suicide.
Develops partnerships with community agencies with regular contact and communication.

Basic Requirements:
To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement.

Education.
Must Have a Master's Degree in Social Work from a school of Social Work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of Social Work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A Doctoral Degree in Social Work may not be substituted for the Master's Degree in Social Work. Verification of the degree can be made by going to the CSWE website to verify if that Social Work degree meets the accreditation standards for a Master of Social Work.
Licensure.
Persons hired or reassigned to Social Worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website.
English Language Proficiency.
Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3J, this part.
Preferred Experience:
1. Do you have at least 1 year of experience working with and/or coordinating services with community programs?
2. Do you have at least 1 year of experience in a mental health leadership role (i.e. Supervisor, Manager, Program Coordinator)?
3. Do you have at least 1 year of post-LCSW, full-time experience providing psychotherapy?
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.

Grade Determination:
Worker Program Coordinators and Manager.
For Social Worker Program Coordinators, grades are determined by the scope of responsibility and the type, size, and complexity of the assignment.
(a) Experience, Education and Licensure.
Individuals assigned as social worker program coordinator or manager must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs in subparagraph (b) below.
(b) Demonstrated Knowledge, Skills, and Abilities
In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring.
Ability to supervise multidisciplinary staff assigned to the program.
Ability to organize work, set priorities, meet multipledeadlines, and evaluate assigned program area(s).
Ability to provide training, orientation, consultation and guidance within clinical specialization of practice.
(c). Assignments.
The KSAs shown above apply to all Social Worker Program Coordinator grade levels.
GS-12 Social Worker Program Coordinators.
Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty, such as Spinal Cord Injury (SCI) coordinators, Visual Impairment Services Team (VIST) coordinators, Homeless Veterans Program (HVP) coordinators, Community Nursing Home Program (CNHP) coordinators. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget.
NOTE:
The Professional Standards Board (a peer-review group) will review the selected candidate's education and experience and recommend the grade and salary at which the candidate will be placed. For internal selectees eligible for promotion, the board will determine grade and HR will determine step in accordance with policy.
Physical Requirements:
Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Social Worker must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the Social Worker or others. Social Worker must be able to pass VA requirements for operation of a government vehicle.
Position requires walking throughout the clinic and/or Medical Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. Position requires travel to meet with community providers, representatives/stakeholders in clinics, medical centers, and VISN offices who provide assistance and support to the HJCR Program and its Veterans.
This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.
