JOB SUMMARY: In accordance with the CHI St. Vincent Health System mission and core values, the medical assistant must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age, gender, and culture of the patients served within the clinic. The individual must demonstrate knowledge of the principles of growth and development over the life span of the patient, including the assessment of data reflective of the patient’s status and age-specific requirements as described in policies, procedures, and performance standards.
Essential Functions
General Care
Liaison between the provider and his/her patients:
Assists provider to remain on schedule with appointments.
Receives, greets, and triages patients in person, telephone, portal, etc.
Schedules specialty appointments, answers questions, and/or determines the need for provider intervention.
Conducts pre-visit planning activities:
Reviews schedule 2-3 days in advance of appointment to prepare visit agenda, identifying care opportunities, document in EHR, and communicate in daily provider huddles.
Rooms and prepares the patient for examination:
Prepares rooms for daily patient care
Take vitals and weight
Reason for visit, medication reconciliation, and allergies
Discuss any recent care transitions (including recent visits to hospital, ER, specialists, etc.)
Ensure all lab and imaging results and/or specialist notes are available for the provider.
Initiate any labs/imaging per standing orders
Assure required patient consent forms are signed (e.g., joint injections, etc.).
Fulfills environmental responsibilities as assigned which may include setting up instruments and equipment according to department protocol, cleaning exam/procedure rooms, instruments and equipment between patient visits to maintain infection control; cleaning sterilizer according to scheduled maintenance program and keeping appropriate records; ordering, sorting, storing supplies; restocking exam/procedure rooms
Populates patient-reported information into EHR templates for specific disease states and preventive visits:
Update patient note including chief complaint, current medications, medical history, allergies, GYN/OB history, surgical history, hospitalization, family history, social history, review of systems, vitals, past results, and treatment plan highlights.
Ensure all quality measures and immunizations are up-to-date.
Orders/administers injections, orders/collects specimens for lab testing, and performs phlebotomy as necessary per standing orders (protocols).
Note: Provider completes history of present illness (HPI) into EMR unless MA is present during exam, per CMS guidelines.
Prepares patient for discharge:
Patient education
Identify opportunities for use of share-decision making aids; document use in EMR.
Review plan of care with patient and answer questions
High-risk patients
Coordinate patient care with Care Management staff, if applicable.
Educate/coordinate the patient regarding Care Management staff and functions
Facilitate the development of Care Plans for high-risk patients
Follow-up care coordination
Scheduling follow-up appointment, provider’s referral information or scheduling patients for testing procedures and providing instructions.
Provides assistance with diagnostic procedures.
Post-visit duties
Patient questions & medication refill requests
Manages inquiries to patients regarding test/lab results and instruction for treatment as directed by the provider.
Processes medication requests/changes/refills per written standing orders (protocols), requiring basic knowledge of medications, dosages, side effects and interactions.
Encourage use of patient portal.
Uses appropriate knowledge and skills in the care & interaction with customers across the age continuum.
Performs care according to hospital / clinic departmental policies & procedures.
Other duties as assigned.
Clerical
Answers and processes patient inquiries according to policy and procedure guidelines.
Distributes to the appropriate destinations.
Follows appropriate protocol for patients in distress such as:
Chest pains
Shortness of breath
Allergic reactions
Stewardship
Support Quality Program initiatives including Patient-Centered Medical Home (PCMH), Comprehensive Primary Care (CPC), Meaningful Use (MU), PQRS, etc.
Assumes responsibility for personal and professional growth through participation the clinic meetings in-service programs, and continuing education programs.
Is honest in using hospital and clinical resources.
Uses time wisely in completing job assignments.
Communication
Demonstrates a professional attitude and approach with patients, families, and all others while ensuring confidentiality of patient and organizational information.
Receives, priorities, documents and responds to incoming inquiries as appropriate.
Information Management
Verifies information accurately
