The Veterans Affairs Texas Valley Coastal Bend Health Care System (VATVCBHCS) is comprised of the Corpus Christi Outpatient Clinic (CCOPC), Corpus Christi Specialty Clinic (CCSC), Harlingen Outpatient Clinic (HOPC), Health Care Center at Harlingen (HCC), Laredo Outpatient Clinic (LOPC), and McAllen Outpatient Clinic (MCOPC). The VATVCBHCS is an ambulatory health care system providing health care in medicine, surgery, psychiatry, and rehabilitation medicine. The VATVCBHCS also operates a 100-patient Home Based Primary Care program and a 200-patient Care Coordination Home Tele-Health Program. Through the utilization of inpatient/emergency department contracts, approximately 2600 inpatient admission and 5800 ER visits are managed by VATVCBHCS. Affiliated with the University of Texas Regional Academic Health Campus, the system has an active ambulatory care program and serves as a teaching and training site for medical residents. The VATVCBHCS is responsible for an enrollee population of over 30,000 veterans and 350,000 outpatient visits annually.

Responsibilities

The Geriatric and Extended Care (GEC) Medical Practitioner will provide high quality patient centered care to Veterans under the different services such as Home Based Primary Care, Hospital in Home, Geri-PACT clinic and Palliative/Hospice care assigned. The medical Practitioner in collaboration with the Medical Directors and Program Managers of assigned services are responsible for the quality of care provided by the Program Managers of assigned services are responsible for the quality of care provided by the program, the productivity of the program and for the development and implementation of initiatives to meet the Valley Coastal Bend Health Care System mission. The medical practitioner will participate in chart reviews, staff meetings, comitees, and the other process improvement initiatives.The medical practitioner will participate in the development and implementations of action plans, performance improvement plans such as a Medical Home and Patient Centric Care, Utilization, Quality and Patient Safety, and Process Improvement (System Redesign). The medical practitioner will put to practice medical care advances and practice standards. The medical practitioner will demonstrate effective communication both down to staff and up to leadership, to include patient outcomes and issue briefs. The medical practitioner will provide coverage to other Geriatric and Extended Care Providers. The medical practitioner will participate in clinical management including direct and indirect patient care: ensure all encounters are closed the same day the visit was made and respond to consults and complete documentation requirements in CPRS in a timely manner including cosigning of students/fellow/resident notes documentation. The medical practitioner will provide clinical input and oversight for patients' treatment plans. The medical practitioner will assure that care is provided in a timely manner and in an appropriate setting, which is in accordance with accepted practices and is evidence based.

Physical Requirements: The Staff Physician for Geriatric and Extended Care position will require moderate lifting, 15-44 pounds, moderate carrying, 15-44 pounds, straight pulling 2-3 hours, walking (2-3 hours), standing (6-8 hours), kneeling 2-3 hours, repeated bending 4-6 hours, working alone, working outside and inside, and working closely with others.

Selected applicants will be required to complete an online on boarding process.

Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.

Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.

Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.

Proficiency in spoken and written English.
