The Clinical Dietitian is the medical nutrition expert responsible for independently planning, developing and directing all nutritional care activities for assigned clients, including those with complex medical and nutritional needs. He/she supports the facility's mission and coordinates nutritional care with other health care providers, the Veteran, the Veteran's family, and the community.

Responsibilities
Duties of this position include but are not limited to:
Completes a comprehensive nutrition assessment to obtain, verify, and interpret data needed to identify nutrition-related problems, their causes, and significance.
Collects data through Veteran/client interview, observation and measurements, the medical record, and the referring health care provider.
Calculates nutritional requirements such as calories, protein, carbohydrates, lipids, and fluid.
Orders diets, labs, and consults per scope of practice/clinical privileges.
Identifies and describes a specific nutrition problem that can be resolved or improved through treatment/nutrition intervention.
Determines intervention strategies to change nutritional intake, nutrition-related knowledge or behavior, environmental conditions, or access to supportive care and services with consideration for cultural and religious beliefs, emotional barriers, desire and motivation to learn, physical or cognitive limitations, and barriers to education to promote Veteran-centered, long-term adherence to a nutrition plan of care.
Assists in the education of dietetic students and interns. Plans learning experiences, teaches, and evaluates performance.
Participates in community projects and education as needed/assigned.
Serves on education and wellness committees as needed/assigned.
Maintains productivity standards and practices effective time management and prioritization of tasks.
Maintains accurate record keeping of daily clinical activities, including information for scheduling and billing.
Assists in achieving compliance with regulatory agency standards including those of formal accrediting bodies (e.g., TJC and CARF) as well as internal and external VA evaluations (e.g., OIG CAP/CBOC, LTCI and AWE/Readiness).
Collaborates with Food Service staff.
Assists with planning menus for regular and modified diets and special events taking into consideration factors such as nutritional requirements, Veteran satisfaction, budget allocation, available manpower, and equipment limitations.
Assists with the review of new products for acceptability on a variety of levels including taste, quality, cost, portion size, and appearance.
Determines need for home care to assure continuity of care and makes appropriate referrals to HBPC, Social Work Service, CBOCs, or other community agencies.
Advises interdisciplinary health care team of nutritional recommendations, goals, and efficacy of nutrition therapy. Discusses pertinent findings and interprets nutritional information which will assist the team in developing comprehensive health care plans.
Assists in Veteran discharge planning; ensures provision of appropriate services and equipment for enteral feeding.

Education:
The individual must have earned a minimum of a bachelor's degree from a U.S. regionally accredited college or university, and fulfilled each of the following criteria:
(1) Completed a didactic program in dietetics accredited by the Commission on Accreditation for Dietetics Education (CADE).
(2) Completed a CADE accredited supervised practice program.
(3) Passed a national examination administered by the Commission on Dietetic Registration (CDR), the credentialing branch of the American Dietetic Association (ADA). Non-registered applicants who otherwise meet the minimum requirements in the basic qualification standard may be appointed as a graduate dietitian under the authority of 38 U.S.C. § 7405 (a) (1) (D). Failure to obtain CDR registration during that period is justification for termination of the temporary appointment. This may result in termination of employment.
Registration:
(1) All applicants must be registered with the CDR.
(2) Non-registered applicants who otherwise meet the minimum requirements in the basic qualification standard may be appointed as a graduate dietitian under the authority of 38 U.S.C. § 7405 (a) (1) (D). Failure to obtain CDR registration during that period is justification for termination of the temporary appointment. This may result in termination of employment.
Grade Determinations
:
(1) GS-7
(a) Experience or Education. None beyond the basic requirements. In addition, the candidate must demonstrate the following KSAs:
(b) Demonstrated Knowledge, Skills, and Abilities
1. Ability to provide nutrition counseling to patients, family members and/or caregivers considering psycho-social issues and age appropriate guidelines, and to communicate effectively with other health care professionals.
2. Ability to plan and organize work consisting of multi-tasks and priorities under general supervision.
3. Basic knowledge of computer operations; software, hardware, terminology, security requirements, and capabilities.
4. Ability to synthesize nutritional assessment components to evaluate nutritional status and utilize principles of medical nutrition therapy using established practice standards and guidelines to deliver nutritional care.
5. Knowledge of basic food production and service which incorporates principles of sanitation, safety, and menu planning within a healthcare environment.
6. Knowledge of educational concepts, methods, training materials, and resources and the ability to implement existing education and training activities for Nutrition & Food Service employees.
(c) Assignments. Employees at this grade are closely supervised and competency with progressively complex tasks is monitored by supervisor. Employees at this grade level serve in a career development position.
(2) GS-9
(a) Experience or Education. At least 1 year of experience at the next lower level that demonstrates the core competencies described at that level, or education equivalent to 2 full years of progressively higher level graduate education, or master's or equivalent graduate degree from an accredited university or college in the field of dietetics or closely related field. In addition, the candidate must demonstrate the following KSAs:
(b) Demonstrated Knowledge, Skills, and Abilities
1. Ability to plan and organize work of considerable difficulty and responsibility under minimal supervision.
(c) For Clinical Dietitian Positions, the following Knowledge, Skills, and Abilities must be demonstrated:
1. Ability to participate in research and performance improvement studies.
2. Knowledge of, and ability to, apply the basic principles of enteral and parenteral nutrition support.
3. Knowledge of the principles and practices of clinical dietetics and medical nutrition therapy in order to plan, develop, and independently coordinate medical nutrition therapy intervention, interdisciplinary care plans, and nutrition education activities.
(3) GS-11
(a) Experience or Education. At least 1 year of experience at the next lower level that demonstrates the core competencies described at that level, or education equivalent to 3 full years of progressively higher level graduate education, or a Ph.D. or equivalent doctoral degree from an accredited university or college in the field of dietetics or closely related field. In addition, the candidate must demonstrate the following KSAs:
(b) Demonstrated Knowledge, Skills, and Abilities
1. Knowledge of basic investigative processes and techniques in order to participate in the design of studies, collect data, interpret findings, and translate results into written and oral communication.
(c) For Clinical Dietitian Positions, the following Knowledge, Skills, and Abilities must be demonstrated:
1. Knowledge of the principles and practices of clinical dietetics and medical nutrition therapy for patients with complex medical conditions in order to plan, develop, and coordinate medical nutrition therapy intervention, interdisciplinary care plans, and nutrition education activities.
2. Knowledge of complex areas of clinical nutrition including management of critically ill and malnourished patients, and ability to apply medical nutrition therapy to patients with multiple co-morbidities.
3. Ability to act as a mentor/consultant for health care providers including registered dietitians, dietetic interns, nurses, physicians, medical students, and allied health professionals.
