Focusing on treating the whole Veteran through health promotion and disease prevention, the Northeast Ohio VA Healthcare System provides comprehensive, seamless health care and social services for more than 112,000 Veterans across Northeast Ohio. With 18 locations of care, including a 674-bed tertiary medical center, 13 outpatient clinics, two community resource and referral centers, a psychosocial rehabilitation and recovery center, a chronic dialysis center and an ambulatory surgery center, the Northeast Ohio VA Healthcare System's quality services are easily accessible to Veterans in 24 counties.

The healthcare system's hub, the Cleveland VA Medical Center, is a national and regional referral center providing a full range of patient care services with state-of-the-art technology, medical education, research, and general acute and psychiatric emergency rooms. The medical center has been recognized for more programs of excellence than any other VA medical center and is home to two VA research centers, the Cleveland FES Center and the Advanced Platform Technology Center.

Responsibilities

The Northeast Ohio VA Healthcare System is recruiting for a Licensed Practical Nurse. The Licensed Practical Nurse will function in Access Management & Care Coordination section in Patient Care Administrative Service. The Licensed Practical Nurse duties include:

Assisting in the planning of the coordination of the patients' care with all interdisciplinary teams.

Completing comprehensive nursing documentation according to standards.

Educating patients about their general care, side effects and recovery process.

Analyzing and appropriately managing consults, laboratory and radiology results and other medical records/documents.

Providing information to physicians or appropriate health care teams in a timely manner.

Identifying ethical dilemmas and appropriately consulting the Nurse Manager/Chief of service or seeking ethics consultation.

Providing indirect nursing care to patients with complex needs as related to consult management, telephone triage, as well as assuming responsibility for nursing action.

Collaborating with other nursing staff/physicians/unit Directors and Service Chiefs regarding Veterans' care and unit activities.

Clinical management of Non-VA Care programs to include traditional Non-VA Care Consults, Patient Centered Community Care (PC3) and the VA Choice Act.

Serving as VA Choice champion to include service planning and education of the program and providing answers to patients relating to the program.

Assuring patient safety standards are maintained with AM&CC.

Assuring continual compliance with Joint Commission and other regulatory standards, policies and procedures.

Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Must be a graduate of school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing.

Must possess a full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.

Preferred Experience: Experience with Consult Management

Grade Determinations:

GS-3: None beyond the basic requirements.

GS-4: Six months of qualifying experience as an LPN or LVN. -OR- Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures. -OR- Graduation from an approved school of at least 24 months duration.

GS-5: Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.

GS-6: Completion of at least 1 year of additional qualifying experience at the GS-5 level or equivalent fully meeting all performance requirements for the GS-5 LPN/LVN.

References: VA Qualification Standards in VA Handbook 5005/3, Part II, Appendix G13.

The full performance level of this vacancy is GS-06. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-03 to GS-06.
