Summary

Embark on a career opportunity as the Primary Care Mental Health (PCMH) Case Manager for the Leavenworth VA Medical Center (VA Eastern Kansas Health Care System-VAEKHCS). EKHCS is a complexity 1c facility and has robust inpatient and outpatient treatment programs including acute medical inpatient, icu/pcu, acute psychiatry/ mental health rehab beds and three community living centers. We offer comprehensive outpatient physical medicine and rehab, mental health, geriatrics and prosthetics. We do extensive community outreach and placement.

This position can be filled with a Social Worker or a Staff Nurse.

Salary Ranges:

Social Worker GS-185: $50, 918 - $80,089

Staff Nurse VN-610: $59,091 - $93,518

Area of Consideration: First area of consideration will be given to NFEE bargaining unit employees at Leavenworth VAMC. Second area of consideration will be given to EKHCS permanent employees. Third AOC will be given to the public (U.S. citizens and U.S. nationals).

Responsibilities

The Primary Care Mental Health (PCMH) Care Manager is assigned to administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns. This is accomplished in collaboration with members of the Vetera n's inpatient medical team, Patient Aligned Care Team (PACT) outpatient clinics, and Community Based Outpatient Clinics (CBOCs); inclusive of Behavioral Health PACT treatment team members in collaboration with Behavioral Health, and other interdisciplinary treatment teams, as appropriate.

Major duties include, but are not limited to:

Provides a thorough assessment which demonstrates critical thinking skills when assessing mental health concerns in a primary care setting and must provide high quality case management, individual and group therapy, clinical treatment, crisis intervention, advocacy, coordination of linkages to other appropriate VA or community service providers/agencies, as needed by the client.

Collaborate with members of the veteran's inpatient medical team and outpatient PACT treatment team members, in collaboration with Behavioral Health, and other interdisciplinary treatment teams, as appropriate.

Assigned administrative responsibility for clinical program development.

Accountable for clinical program effectiveness and modification of service patterns.

Work Schedule: Monday - Friday 8:00 am - 4:30 pm

Qualifications

Basic Requirements for both disciplines: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Basic Requirements for Social Workers Education: Successful completion of master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited.

Licensure: Must be licensed or certified by a state to independently practice social work at the master's degree level.

Grade Determinations: GS-9: In addition to the basic requirements for employment, the candidates must demonstrate the KSAs below: Ability to provide psychosocial treatment to a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological), and the differential influences of the environment, society, and culture.

Ability to work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling skills. Work with more complex problems is done under close supervision. With guidance from the social work supervisor, ability to assess the psychosocial functioning and needs of patients and their family members, and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed.

Basic knowledge of psychosocial treatment modalities and, under supervision, ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques.

Ability to establish and maintain effective working relationships with clients, staff, and representatives of community agencies. Ability to communicate effectively, both orally and in writing, with people from varied backgrounds.

Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology.

GS-11: In addition to the basic qualifications, candidates must complete a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. --OR-- a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs below: 1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.

Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.

Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.

Ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.

Basic Requirement for Nurses Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN

) or The Commission on Collegiate Nursing Education ( CCNE

). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.

Registration: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.

Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
