This position is located at the Milo C. Huempfner Health Care Center in Green Bay, WI. (****This breathtaking, 161,525-square-foot facility employs approximately 300 staff. They care for nearly 15,000 Veterans when you include those who get additional care here from Iron Mountain, Michigan, and the surrounding areas. The Green Bay Clinic offers a vast array of services, just short of overnight stays and an Emergency Department.

Responsibilities

The Clinical Outcomes and Formulary Management (COFM) Pharmacist is a licensed pharmacist whose function is to promote implementation of locally established guidelines and clinical practice guidelines (CPG) and to analyze resulting impact. The incumbent also works in conjunction with local and VISN decisions to optimize the use of pharmaceutical products for the patients served at the Medical Center with emphasis on producing better outcomes at a lower cost.

Major Duties: Works in close association with other Clinical Pharmacy Specialists and ancillary health care professionals, to identify areas for targeting and promotional ideas.

Performs data analysis to measure guideline performance.

Possesses a professional knowledge of drug information, pharmacology, pathophysiology, advanced pharmacotherapeutics, pharmacokinetics, and advanced clinical pharmacy practice in order to provide comprehensive clinical pharmacy services.

Possess extensive pharmaceutical knowledge of various disease states in order to be able to promote developed guidelines and/or optimization of pharmaceutical products.

Works closely with individual providers in converting patients to appropriate medication as indicated.

Prepares written drug and drug class reviews based on a critical evaluation of the drug literature in order to compare and contrast therapeutic regimens and roles for new drugs.

Reviews clinical findings and information related to medication use to form a data base.

Identifies drug use problems, which are reported to the provider responsible for the patient's care.

Designs a therapeutic regimen or plan, including establishing a therapeutic endpoint, recommending an appropriate drug and dosage regimen, and monitoring the outcome of treatment.

Collects data as appropriate and analyze prescribing practices.

Designs and conducts educational programs on drug therapy for physicians, nurses, pharmacists, pharmacy students, pharmacy resident and other health care professionals.

Actively collaborates with physicians and other health care professionals in the design and implementation of drug research protocols, collection of data, and analysis and interpretation of results.

Qualifications

Basic Requirements:

Citizenship: Must be a Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.)

English Language Proficiency: You must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Education: Must be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (TRANSCRIPTS REQUIRED)

OR

Must be a graduate of a foreign pharmacy degree program that meets the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). (DOCUMENTATION REQUIRED)

Licensure: Must possess a full, current, and unrestricted license to practice pharmacy in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or in the District of Columbia. (MUST PROVIDE A COPY)

Grade Determinations: In addition to the Basic Requirements above, applicants must meet the following grade requirements.

SPECIALIZED EXPERIENCE GS-13 LEVEL: You must have at least one (1) year of experience equivalent to the next lower grade level (GS-12) or equivalent, which is directly related to the Clinical Pharmacy Specialist position. The clinical pharmacy specialist (CPS) functions include, but are not limited to: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.

OR

You must have completed 1 year Post Pharm D. ASHP accredited Residency.

OR

Must be completing a 1 year post-Pharm.D. ASHP accredited VA Residency by July 2018.

In addition, you must fully demonstrate the following KSAs:

1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.

2. Expert understanding of regulatory and quality standards for their program area.

3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.

4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.

5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Creditable Experience: Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).

The full performance level of this vacancy is GS-13.

Physical Requirements: This position has been evaluated as a Level 1 in accordance with the Occupational Safety and Health Administration (OSHA) definition of occupational exposure to blood borne pathogens.
