Summary The ACPC\/E plans, develops and manages all Pharmacy Service educational programs and activities; plans, coordinates, evaluates, precepts, schedules and trains pharmacists, pharmacy residents, pharmacy students and other health care professionals who participate in scheduled rotations and patient education sessions. The ACPC\/E is responsible for management of the ASHP-accredited Pharmacy Residency Program, including recruitment, selection, evaluation of residents; serves as Program Director for the residency training program. The ACPC\/E manages the clinical pharmacy programs and supervises the clinical pharmacy staff. The ACPC\/E manages specialized clinical pharmacy services to inpatients and outpatients, to facilitate rational drug therapy and optimize patient care. Work Schedule: Monday-Friday 7:45am- 4:30pm Financial Disclosure Report: Not required Duties Obtains medication histories from patients, provides patient instruction regarding the safe and appropriate use of drugs, and documents findings in the medical record. Develops therapeutic regimens utilizing an assessment of risk\/benefit and cost\/benefit. Makes rounds, attends clinic sessions, and attends patient care conferences in assigned patient care areas. Assists the Pharmacy Chief in achieving compliance to the cost-effective measures on drug use as directed by the VISN. Develops, implements, coordinates and evaluates education programs for pharmacy personnel to ensure the delivery of quality pharmacy services. Coordinates dissemination of information from all pharmacy relevant committees, task forces or teams to the medical staff. Maintains effective channels of communications with persons both within and outside the organization. Direct front line supervisor over Inpatient Pharmacy Supervisor, Outpatient Pharmacy Supervisor, and Pharmacy ADPAC positions. Works with front line supervisors to assure smooth transition of services in all sections and between all sections. Develops and revises policies and procedures for clinical pharmacy services. Develops, coordinates with the Narcotic\/Study Drug Pharmacist, drug investigations in collaboration with the primary investigators of the medical staff. Assumes primary responsibility for the Pharmacy Practice Residency Program by selecting, scheduling, precepting, and evaluating the pharmacy residents during their one-year program.
