Summary
The incumbent will work in the Community Living Center (CLC). The CLC is a 38-bed skilled nursing and rehabilitation facility under LTCI and CMS standards with a spectrum of services ranging from post-acute skilled care and acute rehabilitation to respite, palliative care and hospice. Job duties include both administrative and clinical functions.

Responsibilities
Clinical duties include but are not limited to oversight and facilitation of the process of all admissions to the CLC, including those from acute care facilities; carrying a panel of CLC inpatients in collaboration with nurse practitioners, providing oversight of the care provided by other CLC providers; and providing consultations and interventions to enhance the quality of clinical care in the CLC. After hours on call duties for weeknights and weekends are shared with other JJP VAMC physicians. Coverage of inpatient unit or Urgent Care when provider not available, approximately 8-12 weeks per year.
Administrative duties include but are not limited to monitoring and maintaining quality measures in the CLC; responsibility for survey preparedness and implementation of LTCI and Joint Commission requirements and recommendations; and participation in both CLC and Medical Center committees to identify issues and seek solutions to problems. The incumbent will collaborate with the Associate Chief of Staff for Primary Care, Associate Director of Patient Care Services, and CLC Nurse Managers.
This position reports directly to the Associate Chief of Staff for Specialty Care Services.
Work Schedule:
Full-time, 8:00 am to 4:30 pm, Monday-Friday. Weekend coverage in rotation with other providers.

Qualifications
Basic Requirements
:
* United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
* Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
* Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
* Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
* Proficiency in spoken and written English.
* Physical requirements outlined below.
Preferred Experience:
At least 2 years of experience in a post-acute/long-term care setting is required.
Board-certification/eligibility and/or significant experience in Geriatric Medicine is preferred.
AMDA certification and/or completion of AMDA curriculum in Medical Direction is preferred.
Inpatient Acute Care/Urgent Care experience considered a plus.

Physical Requirements:
This position involves direct patient care. Exposure to surroundings which involve moderate risks or special precautions. Typically involves long periods of standing, walking, bending, stooping, lifting, and positioning of patients, sitting and carrying items such as books, instruments, and other items.
