Responsibilities

The Durham Outpatient Clinics maintains the health and manages the care for patients with a variety of medical problems including hypertension, diabetes, arthritis, cancer, congestive heart failure and other chronic diseases. The clinics are designed to provide comprehensive primary/specialty care services, including gender specific care to male and female veterans. Collaboration exists among providers (attending physicians, mid-level providers), registered nurses, licensed practical nurses, pharmacists, social workers and medical support assistants. The Registered Nurse in this position will serve as a primary care nurse in support of the physician or mid-level provider. Duties will include, but not be limited to, oversight of nursing staff, patient intake, documentation, point of care testing and health screening. The RN will assess, triage, take histories and document conditions and concerns for veterans presenting to the clinic and via the telephone. The RN is responsible for promoting an environment that supports caring and courteous service, safe patient care, and positive working relationships. The clinical nurse will coordinate care through the health continuum using health education, health coaching, and health promotion.

Basic Requirements: *This position requires the completion of a Dependent Credentialing Process (VETPRO)*

Required Qualifications: Citizen of the United States. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Must be proficient in spoken and written English.

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected (Please provide a copy of your transcripts with your application). Applicants are responsible for submitting up-to-date resume and other supporting documents, that will be used to determine salary prior to the closing date of this announcement. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research.

Preferred Experience:

· Minimum of three years of successful clinical practice.

· Prior recent experience in ambulatory care/primary care nursing, including care of patients with mental health and health education needs.

· Previous experience in the care manager role and coordinating care/care delivery.

· Previous experience with telephone advice nursing.

· Demonstrated proficiency in using the nursing process in providing care for clients with complex care needs.

· Previous knowledge/experience with the patient centered medical home practice model.

· Demonstrated ability in the areas of interpersonal relationships, problem solving/critical thinking skills and conflict resolution.

· Ambulatory Care RN Board Certified

· Case Management Certified

· Medical -Surgical Board Certified

· Telephone RN Certified

Physical Requirements: The position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring Veterans and objects may be required. Staff may be required to don protective equipment in isolation situations or operative/invasive procedures. RN is responsible for the reprocessing of reusable medical equipment, according to the medical center policies. They are required to used available safe patient handling lifting devices in all clinical areas. They may also be exposed to Veterans who are combative, secondary to delirium, dementia, or psychiatric disorders.
