The Stratton VA Medical Center, part of the VA Health Care Upstate New York, is located in the heart of Albany. Albany, the capital city of New York, the seat of Albany County, and the central city of New York's Capital District, is roughly 140 miles north of New York City and sits on the west bank of the Hudson River. Opened in 1951, our Medical Center serves 22 counties of Upstate New York, Western Massachusetts and Vermont. In 1990 this facility was named in honor of Samuel S. Stratton, U.S. Congressman for 30 years of the 23rd District, a staunch supporter of veterans and a frequent visitor to our hospital. Patient Centered care is our approach to delivering healthcare to our Veterans. This philosophy gives patients and their families the chance to work closely with their healthcare providers to create more personalized care plans. Patient-centered care also considers the Veterans' personal preferences and values, family situations, cultural traditions, and lifestyles; it keeps them informed and engaged throughout their treatment. The Veteran and family members become key members of the healthcare team and are active participants in making healthcare decisions.

Responsibilities

This position is located in the Primary Care Unit of the Stratton VA Medical Center in Albany, NY. All physicians must possess an active license in a state or territory in the United States, and must maintain privileges at all times at the Albany VA Medical Center to be able to work as a physician. Physicians must be board certified or board eligible in Internal Medicine and maintain professional competency as indicated in their delineation of privileges. All primary care physicians must maintain current BLS certification.

The primary care physician duties will include, but are not limited to:

The primary care provider is an integral member of the PACT teamlet and is expected to work closely and coordinate patient care with other PACT teamlet members, including nursing and clerical staff. Primary care physicians will participate in daily huddles with other teamlet staff and participate in primary care systems redesign projects.

A primary care provider in the outpatient setting is assigned a panel of patients to manage under PCMM guidelines; recommended panel size at the division level is calculated based on patient complexity, support staff and room availability. each provider's panel size is specific to their practiced and setting.

Alternative or collateral duties may include management of a subsection of primary care patients, the subsection may be defined either by a disease status (e.g. spinal cord injury patients), or demographic status (women veterans, OIF/OEF veterans, employees, etc.). Typically these duties are designated at the time of joining but may be assigned in the course of employment in response to new initiatives.

Primary care physicians will have responsibility for supervision of or collaboration with primary care mid-level providers. Physicians are expected to provide clinical oversight and to participate in peer review of other primary care providers through periodic chart reviews.

Primary care physicians will be responsible for supervision, education, and evaluation of residents and medical students in the outpatient primary care setting, as assigned.

Primary care physicians will act as surrogates for absent primary care providers when needed, to include review of CPRS view alerts.

Primary care physicians will regular attend primary care staff meetings, departmental meetings and medical staff meetings.

Typical work hours are Monday through Friday 8am-4:30pm, and a full time provider is required to work a minimum of 80 hours per pay period. May include evenings and/or weekend hours in order to provide extended clinic coverage.

Timely documentation (notes and encounter forms) is necessary in all areas in accordance with facility policy. All administrative work should be completed in a timely fashion.

Clinical reminders should be addressed as medically appropriate. Quality of care should meet professional standards at all times.

All physicians will work within their privileges at all times, unless necessitated by an emergency.

Work Schedule: M-F 7:30am-4:00pm, On-Call Evenings, Nights, Weekends

Physical Requirements: The physician should be physically and mentally capable to perform the duties of the position including examinations, formulate the diagnosis and treatment plan and educate residents. Physical activities include sitting, standing (up to 4 hours) walking (up to 2 hours), heavy lifting (45 pounds or over), reaching above the shoulder, and repeated bending. The incumbent should be emotionally and mentally stable; and capable to perform the duties of the position.

Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.

Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.

Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.

Proficiency in spoken and written English.

American Heart Association Basic Life Support (BLS) Certification:

Appointee must obtain the American Heart Association BLS prior to assuming position.
