This position is located in the Great Plains Area Indian Health Service, Mental Health Program at the Pine Ridge Service Unit in Pine Ridge, South Dakota. The purpose of this position is to assist the patient gain access to needed medical, social, education, and others services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Case management includes assessment of the eligible patient to determine service needs; development of their individualized integrated care plan; referral and related activities to help the patient obtain needed services; monitoring and follow-up; and evaluation.

The IHS is required by law to give absolute preference to qualified applicants who meet the Secretary of the Interior's definition of Indian for appointment to vacancies within the IHS in accordance with established IHS policy as outlined in the Indian Health Manual Part 7, Chapter 3. Responsibilities

Identify patients in need of supportive case management by collaborating with I.H.S staff and other local health care facilities.

Provide documentation of patient contacts/activities through the RPMS and E.H.R computer system on a daily basis.

Assures medication regimen compliance through teaching and interpreting to clients the importance of taking prescribed medications, monitoring usage, and re-filling of prescriptions.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.

BASIC REQUIREMENT(S):

Education: Successful completion of a full 4-year course of study in an accredited college or university leading to a bachelor's or higher degree that included a major field of study or specific course requirements.

Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to perform successfully the duties of the position.

In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-

MINIMUM QUALIFICATIONS:

Basic Requirements for the GS-07 (or equivalent) Grade Level

To qualify at the GS-07 (or equivalent) grade level, applicants must meet one of the following qualification requirements:

1 year of graduate-level education or superior academic achievement

1 year equivalent to at least GS-05 level.

Specialized Experience for the GS-07 (or equivalent) Grade Level: Provides guidance, in-service and orientation regarding Case Management/Discharge Planning. Refers patients to appropriate Mental Health facilities.

Basic Requirements for the GS-09 (or equivalent) Grade Level

In addition to the above Basic Requirements, applicants must meet one of the following requirements:

2 years of progressively higher level graduate education leading to a master's degree or master's or equivalent graduate degree

1 year equivalent to at least GS-07 Grade Level.

Specialized Experience for the GS-09 (or equivalent) Grade Level: Monitor and follows up on the implementation of the written service plan by linking clients with necessary medical providers, assisting in gaining access to needed medical, psychiatric, social, education and others maintaining a contract throughout the treatment plan.

This position has promotion potential to the GS-09 grade level. Promotion to the next grade level is at management's discretion and is based on your demonstrated ability to perform the higher level duties, the continuing need for the higher level duties, and Regulatory/Administrative approval.
