Licensed Practical Nurse, Patient-aligned Care Team
The Licensed Practical Nurse in the Primary Care Clinic is an integral part of the Patient Aligned Care Team (PACT). The PACT LPN works under the direction of an RN and reports to the primary care manager. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life.

is required to possess the knowledge and skills to perform, report, and record assigned nursing care in the Primary Care Clinics, including, but not limited to:
Assists patients with Oxygen therapy
Administers medication to include IV medication if certified
Focuses on clinic visits, exams and treatments with provider
Uses motivational interviewing and health coaching to guide patients in order to promote patient self-management toward individualized life plan of care.
Reviews daily schedules for patient care needs
Assists with triage: messages from patients and face -to-face visits
Processing Clinical reminders with appropriate consults and alerts as indicated
Makes Pre-visit contacts
Check expiration dates of meds and supplies
Assists with orientation: PACT patient orientation and new staff.
Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure.
Adheres to standards of practice and safe technique in preparation and administration of medication.
Demonstrates understanding of age- and population-specific characteristics that affect the health care needs of the Veteran.
Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate.
Based on the patient's goals provides patient and family health education, to include self-management, prevention and wellness.
Demonstrates professional behavior (i.e., respect, civility, ability to accept constructive criticism), effective communication and interpersonal skills.
Adheres to principles of infection control, safety, and universal precautions.
Adheres to all requirements in regard to reusable medical equipment.
Responsible for minor adjustments of the PCP schedule prior to daily huddles (i.e., labs, appointments, hospital/ER discharges, staff shortage).
Assists in the orientation and competency validation of students, peers and clerical staff.
Serves as a resource and advocate in the protection of patient and family rights.
Completes mandatory reviews, including motivational interviewing, in order to promote patient self-management toward individualized life plan of care.
Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles.

Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.

Six months of qualifying experience as an LPN or LVN; or graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or graduation from an approved school of at least 24 months duration.
One (1) year of experience equivalent to the next lower grade level or equivalent.
: Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.

The work requires a high level of active mobility with standing, walking, frequent bending, reaching and lifting. Heavy instrument trays, pieces of equipment and supply packages often weighing as much as 25 pounds must be lifted and carried. The employee regularly pushes and/or pulls loaded carts weighing several hundred pounds. Transport of heavily loaded carts must be negotiated through narrow passages, around corners, up and down inclines or uneven floors, over thresholds and in and out of elevators.
