Summary The Home Based Primary Care RN (HBPC) is a professional nurse. This position will be based in the Indiana Veterans Home in West Lafayette, IN. May float to Indianapolis VA HBPC Office. As progression is made in the nursing profession, it is expected that leadership skills and competencies will be sustained with expertise and broad-based clinical knowledge to effectively implement health promotion, management of acute and chronic illness, and care of the geriatric patient in a home setting. The HBPC nurse will work closely with other members of the HBPC team, including Social Work, Nutrition Care, Physical Rehabilitation, Pharmacy, and Psychology, as well as other healthcare professional in planning and initiating care. The HBPC RN is obligated to ongoing, comprehensive evaluation and improvement of clinical practice and health care delivery systems to meet the needs of Veterans in home-based settings. The HBPC RN provides leadership in delivering holistic care through collaborative strategies with others. The incumbent will report to the unit manager of HPBC Work Schedule: Monday-Friday, 8:00 am- 4:30 pm, On- call Saturday and Sunday, rotating weekend coverage between 8:00 am -12:00 pm Financial Disclosure Report: Not required Duties Duties included but are not limited to: 1. Care Management of homecare patients 2. Assessing patients for suitability of placement on Homecare program 3. Educating patients on homecare and chronic disease processes 4. Work in a collaborative manner with providers, nurse practitioners and managers 5.
