NOTE: Appointment, grade, and salary of Hybrid Title 38 positions are subject to the recommendation of the Professional Standards Board and approval by the Medical Center Director.

The duties and responsibilities of this position are extremely complex due to the multifaceted mission of the Miami VA Healthcare System. The Miami VA healthcare System is a tertiary level of care medical facility accredited by The Joint Commission and affiliations with the University of Miami Miller School of Medicine and Jackson Healthcare System. The Computerized Patient Record System (CPRS) is implemented at this facility requiring the incumbent to balance and coordinate medical record management/ documentation to meet Federal and VA regulations, as well as goals of external accrediting bodies i.e., JCO, CARF and others. The pressing need to comply with VA's and other Federal requirements such as reimbursement from health insurance carriers, performance measures and data capture programs as well as others. Ensures active intra-departmental training program is in place for the medical records staff. Determines and meets training needs of extra- departmental professional, para-professional and non-professional personnel by originating training material, providing orientation to newly assigned interns and residents, participating in in-service programs conducted throughout the hospital. As a technical expert in health information coding matters, provides advice and guidance on a medical records program(s) in relation to such issues as documentation requirements, liability issues, advance directives, informed consent, patient privacy and confidentiality, state reporting, etc... The documentation specialist is expected to strive for the optimal payment to which the facility is legally entitled, but it is deemed unethical and illegal to maximize payment by means that contradict regulatory guidelines, e.g. upcoding, DRG creep, etc. Selection of the principal diagnosis and principal procedure, along with other diagnoses and procedures, must meet the definitions of the Uniform Hospital Discharge Data Set (UHDDS).

POSITION REQUIREMENTS: (Candidates must meet the Basic Requirements, Grade Requirements and Physical Requirements)

I.BASIC REQUIREMENTS:

A. US Citizenship

B. Experience and/or Education

(1) Experience. One year of experience that indicates knowledge of medical terminology and general understanding of health records. Six months of the required one year of experience must have provided the knowledge, skills, abilities, and other characteristics (KSAs) needed to perform MRT work.

OR,

(2) Education. Two years above high School with a minimum of 12 semester hours directly related to MRT work (i.e. courses in medical terminology, anatomy & physiology and introduction to health records).

OR,

(3) Experience/Education Combination. Equivalent combinations of experience and education are qualifying. The following educational/training substitutions are appropriate in this provision for combining education and experience:

(a) Six months of experience that indicates knowledge of medical terminology and general understanding of the health record and one year above high school with a minimum of 6 semester hours of health information technology courses.

(b) Successful completion of a course for medical technicians, hospital corpsmen, medical service specialists, or hospital training obtained in a training program given by the Armed Forces or the U.S. Maritime Service under close medical and professional supervision may be substituted on a month-for-month basis for up to six months of experience provided the training program included courses in anatomy, physiology, and medical record techniques and procedures. Also requires six additional months of experience that indicates knowledge of medical terminology and general understanding of the health record.

II. GRADE QUALIFICATION REQUIREMENTS:

At least one (1) year specialized experience equivalent to a GS-8 Medical Records Technician (Coder) in federal service is required.

Specialized experience is experience that equipped the applicant with the particular knowledge, skills, and abilities (KSA's) to perform successfully the duties of the position, and that is typically in or related to the position to be filled. Examples of specialized experience are: Compiling, reviewing, abstracting, analyzing and interpreting medical data incidental to a variety of patient care and treatment activities; Reviewing the medical record and discussing the case with the clinical staff. Performing admission reviews for specific patient populations to facilitate appropriate clinical documentation criteria and ensures the level of services and acuity of care are accurately reflected in the medical record; Selecting of the principal diagnosis and principal procedure, along with other diagnoses and procedures, must meet the definitions of the Uniform Hospital Discharge Data Set (UHDDS); Assisting in the development of guidelines for data compatibility, consistency, and monitoring for compliance to improve the quality for clinical, financial, and administrative data to insure that all information is fully documented and supported; Facilitating improved overall quality, completeness and accuracy of medical record documentation as well as promoting appropriate clinical documentation through extensive interaction with physicians, other patient caregivers and HIMS coding staff to ensure clinical documentation and services rendered to patients is complete and accurate; etc...

Physical Requirements: Work is mostly sedentary in nature in a medical or clinical setting but must be able to walk, stand, and bend, as required. Lifting of objects ranging from 15-25 pounds may also be required; near vision correctable at 13 to 16 to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish basic colors; ability to distinguish shades of colors; hearing without aid; mental and emotional stability
