This position is located at our Harrisonburg, VA CBOC and is within the Nursing Service of our Martinsburg VA Medical Center. The VA Medical Center is located in the heart of West Virginia's Eastern Panhandle on 175 acres in historic Berkeley County. The Medical Center offers a comprehensive range of services, including inpatient medicine, surgery, mental health, a 178 bed long term care facility, and a 312 bed Domiciliary. Outpatient services include Primary Care, Mental Health, Ambulatory Surgery and clinics for rehabilitation, women's health, audiology and speech, prosthetics, and dental. Patient Care is provided through an integrated primary care concept, and each veteran is assigned to a team of health care providers who follow the patient's care both as an inpatient and an outpatient. The Medical Center has a service area of nearly 130,000 veterans in West Virginia, Maryland, Virginia, and Pennsylvania.

Responsibilities

The ARNP is a registered nurse who has completed the necessary educational preparations in order to: engage in primary health care decision making; obtain advanced competencies and skills for assessing physical and psychosocial health/illness status of individuals, families and groups; promote health maintenance and protection against disease wellness; and, manage the care of patients/clients within the scope of preparation and accountability.

The ARNP uses the Patient Aligned Care Team (PACT) model as a basis to deliver care using the nursing process and evidence-based practice to work collaboratively with the Teamlet (Patient, Primary Care Provider, RN Care Coordinator and Clinical Support technician and other disciplines involved in providing care to the patient) in developing the patient -driven holistic care plan for life. The ARNP is a member of the Teamlet providing diagnosis and treatment.

The ARNP possesses a high degree of knowledge, skill, and competence in acute and chronic disease management. The ARNP provides leadership in the application of the nursing process to client care, organizational processes, and/or system in order to improve outcomes at the program, service, or organizational level. The ARNP is adept at interpersonal skills, working closely with all members of the interdisciplinary team and leading these treatment teams as assigned.

Within the scope of practice, the ARNP is accountable for the provision of care/services to enrolled CBOC patients. These patients include those with poorly controlled chronic illnesses who benefit from care coordination. The CBOC ARNP provides medical direction to the teamlet and recommends adjustments to the treatment plan for the purpose of improving the patient's control of chronic illness. He or she serves as an agent of the supervising physician regarding prescriptive authority. Prescriptive authority will be delineated within the ARNP's individual scope of practice.

Duties/tasks include but are not limited to:

collaboration with the teamlet to analyze various data, phone assessments and treatment plans.

providing direct patient care through review of medical information, collaboration with nurses, and face-to face or telephone contact with veterans. The clinical components include assessing, diagnosing, planning, and prescribing pharmacologic and non-pharmacologic treatment of health problems, health promotion, consultation, and preventive care.

liaison interactions with outpatient primary care providers, inpatient treatment teams, and other disciplines in various settings as needed.

writes orders for medications and diagnostic tests.

interprets test results.

adjust treatment and testing regimens.

consults with other health care providers.

triages patient care issues to expedite timely resolution.

satisfy applicable clinical reminders.

educate staff to meet changing program or service needs and consulting regarding issues such as congestive heart failure, chronic obstructive pulmonary disease, diabetes mellitus, hypertension, obesity standards of care, skin and wound care, pain management, care planning, policy interpretation and clinical issues.

Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN

) or The Commission on Collegiate Nursing Education ( CCNE

). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.

Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Preferred Experience: Primary care experience preferred.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.

Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.

Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.
