Located in historic downtown Charleston, South Carolina, the Ralph H. Johnson VA Medical Center is a tertiary care teaching hospital providing the highest level quality care from cardiology to neurology to primary and mental health care for 70,000 Veterans along the South Carolina and Georgia coast. For the past two years, the Ralph H. Johnson VA achieved an overall annual 5-Star rating according to VA's Strategic Analytics for Improvement and Learning Value (SAIL) model. The Charleston VA is also ranked in the top 10th percentile according to the Healthcare Effectiveness Data and Information Set (HEDIS). HEDIS is an independent review that measures performance of 90 percent of America's health plans and facilities in both the public and private sector on dimensions of care and service. The Ralph H. Johnson VA Medical Center is a center of excellence for robotic surgery and orthopedics, and is the first VA National Tele-Mental Health Hub providing care for Veterans across the U.S. The 149-bed hospital includes six community based outpatient clinics, a 20-bed nursing home, women's health, and the full range of inpatient and outpatient care, including medical and surgical intensive care. Responsibilities

The Patient Aligned Care Team (PACT) RN is a highly skilled professional who is able to work independently to optimize patient care, patient safety and operational efficiency for patients. The focus of the individual's performance encompasses a combination of clinical, administrative, education and research. The PACT nurse must demonstrate performance that is broad enough to impact more than one individual and patient care area. S/he performs at a level that affects several clinics, service line(s) or may impact the entire medical center, outlying clinics, supportive areas and the community.

FUNCTIONS: The PACT RN uses the nursing process an evidence-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate, and Clerical Associate) and expanded Team (family/caregiver, internal and community-bases services involved in providing care to the patient) in developing the patient-driven care plan for life. The PACT RN is a member of the Teamlet team. H/She is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The PACT RN provides patient and family health education with a focus on self-management, prevention, and wellness, based on he patient's personalized health plan/goals.

Through his/her well-developed clinical experience is able to anticipate and implement preventive patient care measures. The PACT is responsible for direct patient care on the PACT team and facilitating he appropriate plan of care. The PACT RN is expected to manage patient care in an astute manner, which promotes positive patient outcomes including the areas of triage, telephone triage, and scheduled and unscheduled patients. He/She possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age specific needs, providing care as considered necessary.

In addition, this RN possesses the knowledge and skills to:

Communicate and interact appropriately and courteously with all internal and external customers

Maintain confidentiality of electronic, written, and/or verbal patient/employee information;

Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment

Maintains BLS certification and all mandatory reviews

Administer, monitor, document and evaluate therapeutic interventions and regimens required of patient presenting for treatment

Initiates AED (CBOCs)

Provides emergency airway management

Demonstrates the ability to perform diagnostic testing such as EKGs, breathalyzer, nebuilzer, Holter monitor (CBOCs), etc.

Perform Venipuncture and IV infusion

Demonstrates ability to utilize various software program including but not limited to corporate databases, VSSC, RAMP, etc.

Function as a charge nurse and assures completion of all clinical and administrative responsibilities timely.

Assesses patients appropriately in all phases of patient care to include but not limited to unscheduled patients, nurse clinic, telephone triage to ensure safe and effective care/treatment

Demonstrates he ability to document appropriately and timely

Interprets laboratory values and analyzes finding of diagnostic tests and responds appropriately in the best interest of the patient.

Ability to work competently in Phlebotomy Lab

Work Schedule: : Monday thru Friday, 7:30 am- 4:00pm. Overtime including Weekends, Evenings and Federal Holidays may be required

Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN

) or The Commission on Collegiate Nursing Education ( CCNE

). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.

Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Preferred Experience: BSN

Two years experience of Emergency Room, Urgent Care, Med/Surg or Primary Care

Highly skilled professional who collaborates with the Primary Care team to coordinate needed services for patients, with emphasis on preventive health care.

Proficient in venipuncture is recommended.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.

Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.

Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.

Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.
