The clinical and program activities assumed under this position are conducted as part of the Mental Health Intensive Case Management (MHICM). MHICM is a joint project of the Behavioral Health and Sciences Council and is administered locally under the direction of the Manager, Behavioral Health and Sciences. The goal of MHICM is to develop, implement, and assess the benefit of a behaviorally-oriented program of psychosocial rehabilitation, intensive case management, and community-based treatment for chronic mentally ill Veterans. Specialized treatment services are provided in various community settings, including the day program, residential care homes, contract nursing homes, and various other community locations. Work is done in the context of an interdisciplinary treatment team, and extensive liaison activities with other interdisciplinary teams are required to provide continuity of care for assigned Veterans in outpatient settings.

Provide intensive case management services to assigned Veterans and significant others. Intensive case management services are characterized by assertive outreach, individualized treatment planning, multidimensional orientation, and intensive delivery of services. Frequency of contacts is increased in times of crisis. Flexibility of Veteran contact (Veterans are seen in any location and with any MHICM team clinician who can provide assistance). A focus on strengthening the patient's ability to address acute and chronic problems. Use of naturalistic community settings for teaching and modeling problem-solving skills. Outreach and assertive provision of services to treatment resistant patients. Mobilization of environmental and organizational supports, including family members, professional, and other interested parties. Work assertively and consistently with inpatient teams to assure continuity of care by providing consultation regarding clinical needs, treatment plans, and case management services for assigned patients. Provide documentation of patient care as prescribed by the MHICM and Medical Center Directives. Also document any problem areas concerning implementation of the program and own activities within the programs, and making suggestions for resolving any such problems consistent with MHICM and Medical Center policies and procedures.

Work Schedule: Monday through Friday, 8:00am to 4:30pm

Basic Requirements:

Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.

Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the advanced practice level. Current state requirements may be found on the OHRM website.Preferred

Experience: Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

Physical Requirements: The physical demands of this position involve active listening, reading, observing and evaluating verbal and nonverbal behavior, intermittent walking and sitting, speaking, writing reports, writing policies and procedures and writing clinical notes. The position requires the use of office equipment such as keyboards, computers, telephones, fax machines and photocopy machines. Light lifting is required up to 35 pounds to lift items such as books, binders, reports and records. Travel is required for this position so a valid driver's license is required.
