Summary The Pain & Palliative Care Pharmacy Program Manager works independently under the Clinical Pharmacy Manager and Chief of Pharmacy, or designee, and keeps them apprised of trends\/problems associated with their assigned areas. The primary responsibility of the Pharmacy Program Manager is the provision of evidence-based pharmaceutical care services through direct interaction with patients and providers. This pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels. The Pharmacy Program Manager functions as part of the Pharmacy team to accomplish department missions and goals. Work Schedule: Monday - Friday, 7:30am - 4:00pm Financial Disclosure Report: Not required Duties Specific duties include, but is not limited, to the following: Clinical: Demonstrates expertise in the areas of geriatrics, pain management, and palliative care. Participates in palliative care multidisciplinary rounds, team meetings, conferences, and other meetings as required for the purpose of making recommendations and providing consultation in all pharmacy-related matters. Designs, implements, and monitors therapeutic drug plans to achieve definite outcomes by direct interactions with patients and providers in assigned areas. Assesses the potential for pain medication non-compliance and develops strategies for non-compliant patients and patients with a history of addiction or potential for addiction. Reviews patient medication regimens for cost-effective drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues.Documents findings per facility and VA policy. Education and Training: Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy residents, pharmacy students, and other medical center personnel as required. Effectively instructs patients and family members in the appropriate use of medications and medical devices. Serves as a preceptor for pharmacy students and\/or pharmacy residents. Maintains a current knowledge of therapeutics and disease state management. Provides timely and accurate responses to drug information inquiries from all customers. Program Management: Coordinates and supervises all pharmacotherapy activities for the Pain & Palliative Care sections, to include: scheduling, reviewing consults, finishing encounters to meet timeliness standards, providing patient care, and assuring coverage of clinical services. Acts as a point-of-contact with Patient Care Services for resolution of time-sensitive nursing\/pharmacy issues. Participates in the medical center and pharmacy service quality improvement programs, including the ongoing professional practice evaluations.
