\t\t\tEXAMPLES OF WORK:\r
\t\t\r
\t\t\r
\t\t\tThis position is within the Developmental Disabilities Division of DHHS. The successful candidate, under general supervision, will be responsible for the following:\r
\t\t\t• Perform intake\r
\t\t\t• Secure assessments\r
\t\t\t• Identify habilitative training needs\r
\t\t\t• Assist in accessing due process procedure\r
\t\t\t• Monitor and evaluate effectiveness\/adequacy of services\r
\t\t\t• Coordinate the development of individual\/family support plans and individual budgets\r
\t\t\t• Ensure medical, legal, and financial service needs are met serving as a liaison for individual\/family\r
\t\t\t• Conduct on-site monitoring visits\r
\t\t\r
\t\t\r
\t\t\tQUALIFICATIONS \/ REQUIREMENTS:\r
\t\t\r
\t\t\r
\t\t\t\r
\t\t\tREQUIREMENTS: Bachelors degree and professional experience in: education, psychology, social work, sociology, human services, or a related field and experience in services or programs for person with intellectual or other developmental disabilities.\r
\t\t\t\r
\t\t\tPREFERRED: Two years experience in services or programs for person with intellectual or other developmental disabilities.\r
\t\t\t\r
\t\t\tOTHER: Valid driver's license or the ability to provide independent authorized transportation.\r
\t\t\t\r
\t\t\r
\t\t\r
\t\t\tKNOWLEDGE, SKILLS AND ABILITIES:\r
\t\t\r
\t\t\r
\t\t\t\r
\t\t\tKnowledge of: current practices in the field of community-based services for persons with intellectual disabilities and other services for persons with developmental disabilities; person-centered program planning; the principles of normalization; provision of habilitation services; positive behavioral supports; statutes and regulations pertaining to delivery of services for individuals with developmental disabilities. Ability to: assess the needs of persons with intellectual or other developmental disabilities; evaluate assessments; determine eligibility; develop and assess individual program plans and individual family support plans; mobilize resources to meet individual needs; communicate effectively to exchange information; develop working relationships with individuals with intellectual or developmental disabilities, their families, interdisciplinary team members, agency representatives, and individuals or advocacy groups; analyze behavioral data; conduct formal assessments; monitor services provided; apply agency and program rules, policies, and procedures; organize, evaluate, and address program\/operational data.\r
