Responsibilities

The GS-6 Licensed Practical Nurse (LPN) is assigned to the Surgery Clinics and accountable to the Nurse Manager in completion of patient assignments and communicating patient responses to the designated Registered Nurse. The incumbent will provide care in a safe, effective, timely and respectful manner. The LPN is expected to accept, support and incorporate the nine guiding principles of the Medical Center and the I-Care values into their daily activities. Their duties should be performed in a manner that promotes quality healthcare, outstanding customer service, and the maintenance of an environment committed to serving the veteran.

The incumbent appropriately prioritizes patient care needs and adjusts plans for care delivery; makes astute observations of subtle, less obvious changes, both physically and emotionally in a patient's condition and accurately and thoroughly documents care delivered. The LPN appropriately seeks assistance and guidance by informing the RN of changes in a patient's condition requiring higher-level intervention. They observe more subtle physical and emotional changes in patients and demonstrate taking appropriate action based on their observations and LPN scope of practice, communicating their observations with the RN and/or attending physician.

The LPN proactively assists in addressing patient's needs for treatments/interventions accurately, according to established policies/procedures and appropriately records the patient's responses to the treatments/interventions received. Assists individuals or groups of patients and their families in the surgery clinics to take an active role in promoting healthy lifestyles for more positive patient outcomes.

Administers prescribed therapies using appropriate judgments and in accordance with medical center policies and procedures; maintains patient airway by suctioning secretions from oral cavity, nasopharynx, nasotrachial, endotracheal tubes, and obtains tracheal aspirate for lab analysis as indicated; performs ostomy care; provides care to patients with various prosthetic devices; performs bladder cathererization as ordered; changes sterile dressings and performs wound care as ordered; prepares and administers medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies.

Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Graduate of a school of practical or vocational nursing approved by the appropriate State accrediting agency and/or the NLN.

Current, full, active and unrestricted registration as a licensed practical or vocational nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia.

Proficient in spoken and written English.

Preferred Experience: Proficiency and competency in wound care including casting, application of wound evacuation systems, assistance with outpatient procedures; physician, clinic and / or surgery experience; knowledge and experience in setting up a sterile field and assisting with procedures; urinary catheters and collection devices; telehealth; broad knowledge base with experience in a variety of patient care settings and with diverse patient population

Grade Determinations:

GS-6: One year of experience that indicates (a) Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. [Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/LVNs and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes.] (b) Demonstrated ability to serve as a responsible member of the nursing team and interact [in an appropriate manner] with patients, family members, professional and [other] supportive personnel [involved in the delivery of] patient care, [incorporating acceptable, established customer service standards into practice. (c) Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN or MD/DO. (d) Knowledge and ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO, and assist in resuscitation procedures in cardiac and/or pulmonary arrest. And, (e) Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action.
