The North Florida/South Georgia Veterans Health System, Social Work Service is currently recruiting for one (1) Social Worker Program Coordinator (ACP-GOCC), at the Gainesville VA Medical Center, FL.

Responsibilities
Responsible for implementing Advance Care Planning (ACP) Group Visit (GV) process and Goals of Care Conversations (GOCC) throughout the North Florida/South Georgia Veterans Health System. They are the ACP-GV diffusion initiative site group facilitator/coordinator.
Provides daily operations and clinical management for the program, to include direct patient care. Is a subject matter expert for understanding related State and Federal laws; documentation requirements, and clinical implementation requirements.
Responsible for the monitoring, reporting and tracking program growth in a national data base.
Attends trainings and calls to advance knowledge.
Consults with a variety of stakeholders on to accomplish the work of the program. Responsible for the programs budget/ fund control points. Uses data for performance improvement efforts to improve the effectiveness/efficiency for the program. Must be a skilled communicator capable of developing effective partnerships within the VA, local, state and other federal agencies, and non-governmental organizations.
Required to travel throughout the patient service area of NF/SG VHS to provide education, direct care to Veterans and family members/caregivers, and conduct outreach/educational meetings. Participates in direct Veteran face-to-face clinic contracts, telephone contacts, Clinical Video Telehealth, Video on Demand contacts and other ways to communication.
Creates educational tools, develops programs and implements training focused on ACP-GV-GOCC. Provides education and in-service training to VA employees, Veterans, families and caregivers, and other governmental and non-governmental community partners. Facilitates Advance Care Planning groups (face-to-face or VANTS telephone line) and educational programs. Distributes educational materials provided by VA, local, state and national organizations.
Skill in the Microsoft Office Suite programs, is essential. Expertise in execution of data management/evaluation is critical. Must exercise mature professional judgment. May also participate in the student training program, new employee orientation, and, serve on facility, VISN, and national committees and workgroups with supervisory approval. Work hours will be set hours and may include night and weekends.

Qualifications
Basic Requirements:
Citizenship.
Be a citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education
. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.
English Language Proficiency
. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
Licensure.
Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. Senior Social Workers are licensed or certified to independently practice social work at an advanced level.
GS-12 Social Worker Program Coordinator
Experience, Education and Licensure
. Individuals assigned as social worker program coordinator or manager must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs in subparagraph (b) below.
Demonstrated Knowledge, Skills, and Abilities

. In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program.

. Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring.

. Ability to supervise multidisciplinary staff assigned to the program.

. Ability to organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s).

. Ability to provide training, orientation, consultation and guidance within clinical specialization of practice.
