The Southern Arizona VA Health Care System is currently recruiting for one (1) Social Workers (SW) to be assigned to Patient Aligned Care Team(PACT) in Primary Care & Community Clinics Care Line (PC&CCCL;). The incumbent has responsibility for providing a full range of social work services to Veterans and families.

Responsibilities
The incumbent reports to the Chief, Primary Care Line. Services include provision of high quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the client. This
is accomplished by functioning as a liaison in collaboration with members of the Primary Care and Non­ Primary Care, Patient Aligned Care Teams. The incumbent is a social worker assigned to the Primary Care Line to provide services to Veterans in Various Primary Care Locations. Duties include but are not limited to the following:
Facilitate timely and appropriate psychosocial assessment, patient and family counseling services, psycho education, case management and may also aid with disposition and coordination of follow-up care for Veterans discharged from an acute care ward with in Southern Arizona VA Health Care System (SAVAHCS) from a community hospital/care facility.
Analyze the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions.
Assist with depression care management as a vital part of Primary Care Mental Health Integration (PCMHI) process.
Work with the Chief and Administrative Officer for Primary Care as well as the Primary Care Management Module Coordinator to provide care for the unassigned Veteran population and request a PACT team let assignment for those eligible for assignment.
Develop/implement and follow-up of treatment plans and outcomes.
Work with the PACT teamlet, Veteran and family members to set achievable treatment goals with the Veteran/family based on a psychosocial assessment.
Implement the treatment plan through methods of, individual, group and family psychotherapy, casework and community organization or other modes of intervention consistent with his/her scope of clinical proficiency.
Educate the Veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker.
Manage a normative workload in his/her area, establishing priorities for timely service delivery to prevent hardship for patient/families and prevent delays in discharge from treatment.

Qualifications
Basic Requirements
:
* United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
* Education:
Must have a Master's degree in Social Work (MSW) from a school of Social Work fully accredited by the Council on Social Work Education (CSWE).
* Licensure:
Must be licensed or certified by a state to independently practice Social Work at the Master's degree level. e.g. LMSW/LCSW
Grade Determinations
: GS-11 LEVEL: Requires a minimum of one year of post-MSW degree professional social work experience in the field of health care. This experience must have been in a clinical setting and must have demonstrated the potential to perform advanced assignments independently. (A clinical setting is a medical center, clinic - medical or psychiatric, residential treatment center, or any other type of facility where social work participates in a collaborative treatment and is identified with the medical profession.) Experience must have been obtained AFTER earning a Master's Degree in Social Work.
